the process - get a real vinyl record made of the track, cut the grooves as deep as you can, create a food grade silicone mould of the record, pour your chocolate (dark) into said mould and let it set/cool down. i would advise keeping your record in the fridge until ready to play - it’ll play the track once before either ruining your record player or destroying the record.
